We are glad you have chosen Sports & Spine Associates to take care of your problem. We evaluate and treat a wide variety of musculoskeletal and peripheral nerve disorders. This includes spinal pain, sports medicine injuries, spinal cord/brain injuries, amputations, and other pain problems / conditions that are unusual or have been difficult to diagnose. Once diagnosed, we direct your care using a variety of methods: self help tips, rehabilitative therapies, judicious use of medications, injections and when necessary referral for surgical intervention. Our goal is to maximize your function while minimizing your pain.
Prior to your appointment you will need to obtain ALL prior X-rays, myelograms, CAT scans, and MRI scans relevant to your pain problem, AND a copy of the actual report by the radiologist. Please bring these (do not mail them) along with any medical records of pertinent evaluations (i.e., EMG/NCS, prior M.D. or E.R. evaluations) with you on the day of your appointment. This will ensure the accuracy of treating your problem, as well as save your time and money.
Enclosed is an extensive medical questionnaire. Please fill it out completely, including the pain diagram. While many of the questions may seem irrelevant to your problem, but complete it to the best of your ability. It is very important that we have as much information possible available to us. This will help us decide how best to treat you as an individual. Bring the questionnaire, your past records and X-rays with you on the day of your appointment.
On the day of your appointment, you will undergo a complete evaluation by your physician and other members of the treatment team. The prior diagnostic studies, medical records and the completed medical questionnaire will be reviewed at that time. Your problem deserves this careful attention, to obtain an accurate diagnosis and thereby initiate the appropriate treatment. Your cooperation with our procedures will help to ensure continued excellence of care towards your condition.
Please be on time for your appointment. Please come 15 minutes early for your 1st appointment. This is necessary to complete insurance information, to create a completed chart for the doctor and clarify any other administrative details. We request that you call us to let us know if you will not be able to come to a scheduled visit at least 24 hours in advance. There are many patients on a waiting list that would love to have your appointment. Also, please try to call if you are running late. Although medical emergencies and other factors unique to a medical practice may slow us down, we will work our hardest to be on time as well. If not, your problem will still be given the attention it warrants.
