Office visits are by appointment only. You may schedule an office visit by calling 512-358-0500 or Fax 512-358-0520. Our staff is trained to assess your problem and will assist you in scheduling a convenient time. Due to our high volume of calls, you may be placed on hold temporarily. Please understand that we deal with a wide variety of acute and chronic pain conditions, of different degrees of severity, that need to be triaged differently. That takes nothing away from our attention to your problem. Be assured we will make every effort to accommodate your needs. While an appointment may be given for several weeks later, many of those appointments may be moved up into cancellation slots or blocks of time that had been set aside for other procedures.
Please feel free to down load the patient demographic sheet from the website. Information from this will need completed in order to schedule an appointment. You may fax this, along with a copy of the front & back of your insurance card and a picture I.D. This will save time when calling to make an appointment. Additionally, you may download a copy of the patient questionnaire to complete prior to your appointment.
This WEB Site is only for education of our patients. Unfortunately, we unable to respond to inquiries, based on their volume.

Our office participates in numerous PPO and indemnity insurance plans. If we are a provider under your plan, you must pay any deductible or any co-pay under your contract at the time of your visit. All non-insured or personal injury patients also make payment at the time of service, unless other arrangements have been made in advance. Payments should be made in the form of cash or check or credit cards. Your basic charge will vary depending on the severity and complexity of the medical problem and/or time spent. Certain procedures may not be covered under your plan. Make sure you verify your benefits with your insurance plan. If a procedure that is part of your treatment is not covered by your insurance plan, and you choose to proceed with that treatment, payment is required in advance.
Any PPO patients whose insurance plans require a preauthorization or referral number to see us are responsible for assuring that the necessary authorization for the visit has been obtained from your Primary Care Physician at least 24 hours in advance. If this is not available, we will have to reschedule your appointment. PPO patients should be aware that if we refer you for a treatment, diagnostic study or a physician consultation, we do our best to keep it "in network". This information changes too frequently; therefore the ultimate responsibility for determining whether a location or doctor is on your plan is yours. Many of the same insurance plans have different contracts with different employers. It may be helpful for you to bring your provider manual to your visit. (Even this is not always accurate)
